Daily Medications
All Grades:
If your child requires daily administration of prescription medication
in school, then you must fill out and sign the form entitled
"Physicians Order for Dispensing Medication" and have it on file in the
Health Office.
Grades K - 5: If your child requires daily medication or over the counter medication in school, then you must fill out and sign the form entitled "Physicians Order for Dispensing Medication" and have it on file in the Health Office.
Grades 6 - 12: If your child will require over the counter medication during school, then you must fill out and sign the form entitled "Medication Information and Consent for Non-Prescription Medication"" and have it on file in the Health Office.
